Roles & Responsibilities
- Provide compassionate, hands-on occupational therapy services to residents in the skilled nursing facility
- Implement treatment plans designed by the OTR and document resident progress accurately
- Assist with activities of daily living training and recommend adaptive equipment to maximize independence
- Collaborate closely with nursing, therapy team members, and families to coordinate care
- Educate residents and caregivers on functional strategies and safe transfer techniques
- Maintain accurate, timely documentation in the EMR and support quality improvement initiatives
Qualifications
- Current COTA (Certified Occupational Therapy Assistant) credential
- Michigan licensure or eligibility to obtain Michigan licensure (assistance available)
- Experience or comfort working in a SNF/rehab setting preferred
- BLS/CPR certification or willingness to obtain upon hire
- Strong assessment, documentation, and patient education skills
- Reliable, professional, and able to work effectively within a multidisciplinary team
Education
- Associate's degree in Occupational Therapy Assistant or equivalent accredited program
Knowledge & Skills
- Skilled in ADL training, therapeutic exercises, and use of adaptive equipment
- Knowledge of SNF rehabilitation workflows and care plans
- Excellent communication and interpersonal skills with residents and families
- Comfortable with EMR documentation and progress note best practices
- Ability to adapt interventions to meet individual resident goals and safety needs
