Roles & Responsibilities
- Provide compassionate bedside care and monitor resident condition with attention to detail
- Administer medications and treatments safely and accurately
- Document assessments, interventions, and resident responses in the medical record
- Collaborate with interdisciplinary team members to ensure coordinated care
- Educate residents and families about care plans and health maintenance
- Follow infection control protocols and maintain a safe care environment
- Respond effectively to emergency situations and escalate concerns appropriately
Qualifications
- Current Licensed Practical Nurse (LPN) credential, active and in good standing
- Basic Life Support (BLS) certification
- Ability to pass Trinity specialty competency testing and license/certificate verification
- Completed pre-employment health requirements including fit test, 2-step PPD or IGRA, and required immunizations
- Successful completion of Trinity 10-panel drug screen and physical/health statement
- Strong communication skills and a team-oriented attitude
- Previous LPN experience in long-term care or similar settings preferred
Education
- Graduate of an accredited Practical Nursing program (LPN certificate or diploma)
Knowledge & Skills
- Clinical assessment skills and basic nursing interventions
- Medication administration and safe documentation practices
- Familiarity with infection prevention and control standards
- Comfort using electronic medical records and charting tools
- Time management and ability to prioritize in a busy setting
- Effective communication with residents, families, and care teams
