Roles & Responsibilities

• Provide compassionate, person-centered nursing care to Mercy LIFE participants in the per diem pool
• Administer medications, monitor vital signs, and perform routine clinical assessments
• Document care accurately and timely in the clinical record system
• Collaborate with the interdisciplinary care team to coordinate participant needs and plan of care
• Complete Trinity specialty competency testing and skills checklists to maintain clinical excellence
• Maintain required immunizations, drug screens and health screenings as part of compliance
• Adhere to facility policies, safety protocols, and infection control practices

Qualifications

• Current, active LPN (Licensed Practical Nurse) license — Pennsylvania licensure required or ability to obtain
• BLS (Basic Life Support) required
• ACLS (Advanced Cardiac Life Support) required
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Proof of required immunizations and ability to pass Trinity 10 Panel Drug Screen and background checks
• Professional communication skills and ability to work independently in a per diem setting

Education

• Graduate of an accredited Practical Nursing/LPN program and active LPN license

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with electronic health record (EHR) documentation
• Knowledge of infection control, basic wound care and chronic care management
• Ability to follow established care plans and escalate concerns appropriately
• Good time management, organization, and teamwork skills
• Customer-service mindset with empathy for older adults and their families
