Roles & Responsibilities
- Provide compassionate, person-centered nursing care to Mercy LIFE participants in the per diem pool
- Administer medications, monitor vital signs, and perform routine clinical assessments
- Document care accurately and timely in the clinical record system
- Collaborate with the interdisciplinary care team to coordinate participant needs and plan of care
- Complete Trinity specialty competency testing and skills checklists to maintain clinical excellence
- Maintain required immunizations, drug screens and health screenings as part of compliance
- Adhere to facility policies, safety protocols, and infection control practices
Qualifications
- Current, active LPN (Licensed Practical Nurse) license — Pennsylvania licensure required or ability to obtain
- BLS (Basic Life Support) required
- ACLS (Advanced Cardiac Life Support) required
- Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Proof of required immunizations and ability to pass Trinity 10 Panel Drug Screen and background checks
- Professional communication skills and ability to work independently in a per diem setting
Education
- Graduate of an accredited Practical Nursing/LPN program and active LPN license
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Comfortable with electronic health record (EHR) documentation
- Knowledge of infection control, basic wound care and chronic care management
- Ability to follow established care plans and escalate concerns appropriately
- Good time management, organization, and teamwork skills
- Customer-service mindset with empathy for older adults and their families
