Roles & Responsibilities
- Provide compassionate, resident-centered nursing care as an LPN within the LIFE program
- Administer medications and treatments safely and document accurately
- Monitor and report changes in participant condition to the care team
- Complete required competency testing and skills checklists to maintain high standards
- Participate in infection control practices, including respirator fit testing and immunization protocols
- Collaborate with interdisciplinary team members to support participant independence and quality of life
- Assist with care plan implementation and timely, clear charting of interventions and outcomes
Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing (required)
- Basic Life Support (BLS) certification (required)
- Ability to complete license/certificate verification and required Trinity competency testing
- Willingness to complete Trinity skills checklist, respirator fit test, 10-panel drug screen, and health screenings
- Up-to-date immunizations or acceptable titers: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
- Current physical/health statement as part of onboarding
- Prior LPN experience in long-term care, home/community care, or similar settings preferred
Education
- Graduate of an accredited LPN/Vocational Nursing program or equivalent
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Strong documentation and clinical observation skills
- Knowledge of infection control and OSHA/respiratory protection practices
- Comfortable working independently in community-based care settings
- Excellent communication and teamwork skills with residents and multidisciplinary teams
- Ability to complete competency assessments and follow established clinical protocols
