Roles & Responsibilities

• Provide compassionate, resident-centered nursing care as an LPN within the LIFE program
• Administer medications and treatments safely and document accurately
• Monitor and report changes in participant condition to the care team
• Complete required competency testing and skills checklists to maintain high standards
• Participate in infection control practices, including respirator fit testing and immunization protocols
• Collaborate with interdisciplinary team members to support participant independence and quality of life
• Assist with care plan implementation and timely, clear charting of interventions and outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing (required)
• Basic Life Support (BLS) certification (required)
• Ability to complete license/certificate verification and required Trinity competency testing
• Willingness to complete Trinity skills checklist, respirator fit test, 10-panel drug screen, and health screenings
• Up-to-date immunizations or acceptable titers: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
• Current physical/health statement as part of onboarding
• Prior LPN experience in long-term care, home/community care, or similar settings preferred

Education

• Graduate of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Strong documentation and clinical observation skills
• Knowledge of infection control and OSHA/respiratory protection practices
• Comfortable working independently in community-based care settings
• Excellent communication and teamwork skills with residents and multidisciplinary teams
• Ability to complete competency assessments and follow established clinical protocols
