Shift Type: Monday-Friday or Tuesday-Saturday, 8-hour shifts

Candidate Pay Rate: $77 per hour

Roles & Responsibilities
- Conduct start of care assessments and develop care plans
- Perform recertification visits and resumption of care
- Provide routine home health nursing visits
- Manage patients with complex conditions including CHF, COPD, and wounds
- Educate patients and families on disease management
- Collaborate with home health aides and interdisciplinary team
Qualifications
- Active RN license in California
- Current BLS certification
- Valid driver's license and reliable transportation
- Auto insurance coverage
- HomeCare HomeBase (HCHB) experience preferred
- Minimum 1 year of home health experience
Knowledge & Skills
- Knowledge of home health regulations and OASIS
- Ability to manage wound care, IV-PICC lines
- Strong clinical assessment skills
- Excellent communication and patient education skills
- Time management and ability to complete 5-6 visits per day
