Roles & Responsibilities

• Lead and coordinate the Long-Term Care QAPI program to drive measurable quality improvements
• Plan, organize, and direct quality assurance activities, including performance monitoring and development of action plans
• Conduct chart audits, data analysis, and clinical outcome reviews to identify trends and opportunities
• Facilitate accreditation readiness and participate in regulatory surveys to ensure compliance with CMS and other bodies
• Collaborate with administration, providers, leaders, and staff to coach, educate, and integrate quality practices across the facility
• Develop, review, and revise policies, procedures, and facility-wide assessments to maintain best practices
• Investigate incidents, support Datix and facility-reported incidents, and perform root cause analyses with the Risk Manager
• Organize and facilitate quality committee meetings, maintain minutes, and report findings and recommendations to leadership
• Provide cross-coverage for QM team roles as needed and participate in rounding and interdisciplinary teamwork

Qualifications

• Current full, unrestricted Registered Nurse (RN) license in Alaska (agency staff must have active license; direct hires must have applied before start)
• Certified Professional in Healthcare Quality (CPHQ) or QAPI Certified Professional (QCP) or other Long-Term Care quality certification obtained within the last 3 years
• Basic Life Support (BLS) certification (American Heart Association)
• Clinical Competency Assessment - SEARHC
• Minimum 2 years experience monitoring quality and regulatory requirements, planning, and coordinating process changes
• Proven leadership in facilitating cross-functional teams and driving collaborative improvement
• Preferred: hands-on Long Term Care experience

Knowledge & Skills

• Knowledge of accreditation requirements across long-term care, acute, ambulatory, and behavioral health settings
• Strong working knowledge of patient care principles, standards of care, and infection control
• Practical understanding of quality improvement concepts and QAPI methodology
• Skill in data analytics: collection, analysis, and presentation of quality metrics
• Experience with medical chart review and extraction of clinical data
• Familiarity with incident reporting systems such as Datix and risk management processes
• Excellent verbal and written communication, coaching, and education skills
• Proficiency with Microsoft Office (Word, Excel, PowerPoint) and comfort learning facility systems
• Ability to lead group processes, prioritize work, and translate data into actionable plans
