Roles & Responsibilities
- Deliver personalized physical therapy treatments that help residents regain strength and independence
- Collaborate with an interdisciplinary team to create meaningful care plans
- Assess patient progress and adjust therapy techniques to maximize outcomes
- Educate patients and families on safe exercise routines and home modifications
- Document treatment sessions accurately to support continuity of care
Qualifications
- Active Physical Therapist license in Tennessee
- Doctor of Physical Therapy (DPT) degree or equivalent
- New graduates are welcome to apply
- Strong communication and interpersonal skills
Education
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
Knowledge & Skills
- Skilled in geriatric rehabilitation and fall prevention
- Comfortable working in a Skilled Nursing Facility setting
- Ability to adapt therapy plans for diverse patient needs
- Team-oriented mindset with a passion for patient-centered care
