Candidate Pay Rate: $77 per hour

Roles & Responsibilities
- Provide compassionate home health care to recently discharged patients
- Educate patients and families on managing CHF, COPD, and wound care
- Perform start of care, recertification, and routine visits
- Monitor IV-PICC lines and wound vacs with expert attention
- Help reduce hospital readmissions through proactive care
- Document visits using HomeCare HomeBase system
Qualifications
- Active RN license in California
- BLS certification required
- Valid driver's license and auto insurance
- HomeCare HomeBase experience preferred
- Home health experience a plus
Education
- Associate or Bachelor's degree in Nursing
Knowledge & Skills
- Patient education for respiratory and cardiac conditions
- Wound care and wound vac management
- IV-PICC line monitoring
- Home health documentation systems
- Building trust with patients in their own homes
