Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:30 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Coordinate seamless care transitions for patients across the continuum
- Collaborate with physicians and interdisciplinary teams to optimize discharge planning
- Utilize Midas and Meditech systems to document and track patient progress
- Advocate for patients while ensuring appropriate resource utilization
- Manage complex discharge planning needs with compassion and expertise
Qualifications
- Active RN license in Florida or compact license
- Minimum 1 year of case management experience
- Midas experience required
- Meditech experience required
- Strong communication and organizational skills
Knowledge & Skills
- Proficient with Midas and Meditech electronic systems
- Expertise in complex discharge planning processes
- Ability to coordinate care across multiple units
- Patient ratio experience of 25:1 or similar
- Collaborative approach with RN and Social Work case managers
