Contract Type: Travel Contract

Shift Type: 8-hour day shifts, Monday-Friday or Tuesday-Saturday (preferred), schedule completed by previous day

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide comprehensive home health nursing care to patients recently discharged from the hospital
Manage complex conditions including CHF, COPD, and wound care
Perform IV therapy and PICC line management
Educate patients and families on disease management and self-care
Complete start of care, recertification, resumption of care, and routine visits
Coordinate care with interdisciplinary team and home health aides

Qualifications

Current RN license in the state of California
BLS certification from the American Heart Association
Valid driver's license and current auto insurance
Minimum of 1 year home health nursing experience preferred
Experience with HomeCare HomeBase (HCHB) preferred
Strong clinical assessment and critical thinking skills

Knowledge & Skills

Wound care and wound vac management
IV therapy and PICC line care
CHF and COPD management
Patient education and motivational interviewing
Proficiency in HomeCare HomeBase (HCHB)
