Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with interdisciplinary care teams to ensure holistic patient care
Educate patients and families on exercises and strategies for independence
Document patient progress accurately using HomeCare/HomeBase EMR

Qualifications

Active North Dakota Physical Therapy license (required)
Current BLS certification (required)
Valid driver's license and auto insurance (required)
At least 1 year of recent PT experience, preferably in home health
Willingness to travel within a 60-mile radius of Valley City

Knowledge & Skills

Skilled in treating post-surgical, fall recovery, and chronic condition patients
Proficient with HomeCare/HomeBase or similar EMR systems
Strong communication and patient education abilities
Ability to work independently without on-site supervision
