Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate and lead discharge planning to ensure safe, timely transitions of care
- Collaborate with interdisciplinary teams (social services, MDs, therapists) to create and execute patient-centered plans
- Perform needs assessments and order DME to support patient home readiness
- Document thoroughly and accurately in Epic to maintain continuity of care
- Manage utilization review activities and follow CMS guidelines
- Float between hospital areas (including Colby and Pacific campuses) to meet patient needs
- Provide high-quality acute care case management for patients across specialties
Qualifications
- Active RN license required (pending licenses accepted); MSW candidates may be considered for social work roles
- BLS certification required
- Minimum 1 year of acute care experience (Discharge Planning / Care Coordination experience required)
- Experience with Epic charting system is required
- Comfortable with patient ratios up to approximately 1:25 during day shifts
- Willingness to float across units and work all areas of the hospital, including multiple campuses (~2 miles apart)
- Availability to work every-other-weekend and attend a 30-hour orientation
- Non-local candidates preferred (locals not accepted); include all RTO at time of submittal
Education
- Active RN licensure (education per RN licensure requirements) or MSW for social work candidates
Knowledge & Skills
- Discharge planning and care coordination best practices
- Case management and utilization review processes
- CMS regulations and compliance knowledge
- Epic electronic charting expertise
- Needs assessment and ordering DME
- Plan of care development and execution
- Acute care experience across Med Surg, ED, ICU and longer length-of-stay (LLOS) populations
- Strong communication, prioritization, and interdisciplinary collaboration skills
