Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Coordinate comprehensive care plans for patients across the healthcare continuum
- Collaborate with physicians and interdisciplinary teams to ensure seamless transitions
- Advocate for patients while optimizing resources and outcomes
- Monitor progress and adjust care strategies to meet individual needs
Qualifications
- Active RN license in Texas or compact state
- Minimum 1 year of nursing experience
- Case management or utilization review experience preferred
- Strong communication and organizational skills
Knowledge & Skills
- Patient advocacy and care coordination
- Discharge planning expertise
- Knowledge of healthcare regulations and payer requirements
- Ability to work collaboratively in fast-paced environments
