Candidate Pay Rate: $59.5 - $59.5 per hour

Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans that improve function
Deliver skilled therapy to enhance mobility, strength, balance, and independence
Document evaluations, treatment sessions, and patient progress accurately and in a timely manner
Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
Educate patients and caregivers on therapeutic exercises, safety, and mobility strategies
Maintain organized, safe, and compliant treatment areas
Ensure adherence to facility policies, procedures, and state regulations

Qualifications

Active California Physical Therapist (PT) license (required)
Prior SNF or rehabilitation experience preferred
Strong clinical judgment with excellent communication and documentation skills
Ability to work independently and as part of a collaborative team
Availability to commit to the full 13-week contract

Knowledge & Skills

Patient evaluation and assessment techniques
Individualized treatment planning and intervention
Improving mobility, strength, balance, and functional independence
Accurate and timely clinical documentation
Interdisciplinary collaboration and clear communication with team members
Patient and caregiver education for home safety and therapeutic carryover
Familiarity with facility policies and state regulatory requirements
