Roles & Responsibilities

• Perform thorough physical evaluations to assess patient abilities and needs
• Design and implement individualized treatment plans that promote mobility and independence
• Deliver therapeutic interventions to improve strength, balance, and function
• Educate patients and families on safe movement, exercises, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and care teams to coordinate care
• Monitor patient progress and adjust treatment plans to achieve the best outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and care plans
• Ensure care plans comply with facility policies and regulatory best practices

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California
• Experience in a skilled nursing facility or similar healthcare setting preferred
• Strong knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (accredited program required)
