Roles & Responsibilities

• Perform thorough patient evaluations and develop individualized treatment plans
• Deliver hands-on physical therapy interventions to improve strength, mobility, and function
• Educate patients and families on therapeutic exercises, safe movement, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and care teams to coordinate care
• Monitor progress and adjust treatment plans to achieve the best outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and treatment plans
• Ensure care plans meet regulatory requirements and facility best practices

Qualifications

• Licensed Physical Therapist in the state of California (or eligible for licensure)
• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Experience in a skilled nursing facility or post-acute care setting preferred
• Knowledge of therapeutic techniques, modalities, and adaptive equipment
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of a multidisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
