Roles & Responsibilities

• Provide thorough evaluations and individualized treatment plans for outpatient diagnoses
• Deliver hands-on manual therapy and targeted therapeutic exercise programs
• Perform gait, balance and pulmonary rehabilitation training to improve function and safety
• Manage post-operative care including joint replacement and tendonitis rehabilitation
• Collaborate with PTAs and interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on home programs, injury prevention and self-management
• Document timely and accurate evaluations, progress notes and discharge summaries in CPSI EMR
• Support fall prevention initiatives and community-focused rehab programs

Qualifications

• Doctorate in Physical Therapy (DPT) — required
• Minimum 2 years of current PT experience (new grads will not be considered)
• Active New Hampshire Physical Therapy license or compact (must be uploaded with profile)
• Vermont PT license will be pended upon offer; must apply within 24 hours of offer acceptance
• Current American Heart Association BLS certification (required)
• Previous outpatient rehab experience required; travel experience strongly preferred
• Comfortable treating the full spectrum of outpatient diagnoses and flexible to work inpatient as needed
• Traveler must have own vehicle (small community without public transport) and must not reside within 50 miles of the facility
• Provide an updated CAQH and NPI after start; include supervisory signed reference from the past 2 years when submitting

Education

• Doctorate in Physical Therapy (DPT) from an accredited program — required
• Graduate of an accredited school of Physical Therapy — required

Knowledge & Skills

• Skilled in manual therapy techniques and therapeutic exercise prescription
• Experience with gait and balance assessment and training
• Knowledge of orthopaedic, neurological and pulmonary rehabilitation principles
• Proficient documentation skills and familiarity with CPSI EMR (highly preferred)
• Ability to supervise and collaborate with PTAs
• Strong patient education and communication skills
• Competence in treating post-operative and joint replacement patients
• Ability to manage a busy caseload (typical 8 patients/day Mon–Thu, 6 on Friday)
