Roles & Responsibilities

Assess patients' mobility, pain, and functional needs to create individualized treatment plans that restore independence
Deliver therapeutic interventions and mobility training to reduce fall risk and improve daily function
Document evaluations, progress notes, daily treatments, discharges, and insurance communications accurately and promptly
Coordinate with the rehab team, physicians, and insurance companies via phone and documentation to support patient care and discharge planning
Support a safe, clean treatment environment by assisting with treatment room cleaning and basic facility tasks as needed
Perform other duties as requested to keep patient care running smoothly

Qualifications

Current Physical Therapist license in Wisconsin (or pending with clear timeline) — licensure required to practice
Experience evaluating and treating patients with movement dysfunction and pain; comfort with discharge planning and insurance communication
Strong documentation skills and ability to write clear evaluations, progress notes, and daily notes
Excellent communication skills for collaborating with patients, families, providers, and insurers
Comfort participating in light treatment-room upkeep and team-oriented duties

Education

Graduate of an accredited physical therapy program (DPT preferred)

Knowledge & Skills

Patient assessment and evaluation
Treatment planning and therapeutic interventions
Pain management techniques and mobility training
Fall risk assessment and prevention strategies
Thorough clinical documentation (evaluations, progress notes, discharges)
Experience coordinating with insurance for authorizations and communicating approvals/denials
Effective phone and interprofessional communication
Team-based rehab collaboration and basic environmental cleaning tasks
