Roles & Responsibilities

• Provide skilled physical therapy services to home health patients within a 60-mile radius of Valley City
• Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Document patient progress accurately using HomeCare/HomeBase EMR system
• Educate patients and families on therapeutic exercises and home safety
• Maintain a caseload of 5-7 patients per day, delivering high-quality care

Qualifications

• Active ND Physical Therapy License required
• BLS certification from AHA or equivalent
• Valid Driver's License and current Auto Insurance
• Minimum 1 year of physical therapy experience, preferably in home health
• Previous travel experience not required, but a sense of adventure is welcome

Knowledge & Skills

• Strong assessment and clinical reasoning skills for diverse patient populations
• Proficient in home health documentation and EMR systems like HomeCare/HomeBase
• Excellent communication and patient education abilities
• Ability to work independently with remote supervision
• Time management skills to handle a daily caseload efficiently
