Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments to determine patient needs
Develop and implement individualized care coordination plans
Collaborate with interdisciplinary teams to ensure seamless care transitions
Advocate for patients and families to access community resources
Provide crisis intervention and support for patients under 5150 holds
Document all interactions and maintain compliance with regulatory standards

Qualifications

Current California LCSW license
BLS certification required
5150 certification in California (training and exam completion)
Prior Dignity Health experience is required
Minimum 1 year of experience in care coordination or related area

Education

Master's degree in Social Work from an accredited program

Knowledge & Skills

Expertise in mental health assessment and crisis intervention
Strong knowledge of community resources and referral processes
Excellent communication and collaboration skills
Ability to work effectively in a fast-paced healthcare environment
Proficiency in electronic health records (EHR) systems
