Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care visits, recertifications, and resumption of care for home health patients
Manage a daily caseload of 6-6.5 points, including regular visits and admissions
Collaborate with clinical coordinators and sister agencies for seamless patient care
Provide patient education, safety evaluations, and hospice support visits as needed
Document patient progress and coordinate care with the interdisciplinary team

Qualifications

Active Physical Therapy license in North Dakota
BLS certification (American Heart Association)
Home health experience and OASIS training preferred
Valid Driver's License and auto insurance
Willingness to travel within a 50-mile radius
First-time travelers welcome - we provide support!

Knowledge & Skills

Expertise in home health and hospice care principles
Proficient in OASIS documentation and point-of-care systems
Strong clinical assessment and treatment planning skills
Excellent time management and ability to work autonomously
Computer proficiency for electronic medical records
