Physical Therapist - Home Health
Location: Santa Maria, California
Start Date: 06-16-2026
End Date: 09-15-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible, every 3rd weekend)
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Provide home-based physical therapy to patients in assigned geographical region
- Develop and implement individualized treatment plans for ortho, stroke, joint, and spinal injury patients
- Complete OASIS assessments and documentation accurately and timely
- Collaborate with interdisciplinary team including rehab aides and OTA/PTA staff
- Educate patients and families on home exercise programs and safety
Qualifications
- Licensed Physical Therapist in California (or eligible)
- Minimum 1 year of PT experience, home health setting preferred
- Experience with OASIS documentation required
- Current BLS certification
- Ability to travel within Santa Maria and Arroyo Grande areas
Knowledge & Skills
- Strong clinical skills in orthopedics, neurology, and geriatrics
- Proficiency with Cerner EMR
- Excellent communication and patient education skills
- Ability to work independently and manage a caseload
