Shift Type: Flexible shifts, majority of work hours are Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes
Develop and implement individualized treatment plans for orthopedic, stroke, and spinal injury patients
Collaborate with rehab aides and OTA/PTA staff
Complete OASIS documentation as mandated by government regulations
Cover assigned geographical region

Qualifications

Active Physical Therapist license in California
Minimum one year of PT experience
Experience with OASIS documentation
BLS certification required
Home health experience preferred

Knowledge & Skills

Proficient in Cerner EMR
Experienced in orthopedic and stroke rehabilitation
Strong documentation skills with OASIS
Excellent communication and interpersonal skills
