Roles & Responsibilities

• Conduct comprehensive home health evaluations for orthopedic and geriatric patients
• Develop and implement individualized physical therapy treatment plans
• Provide skilled interventions including therapeutic exercises and manual therapy
• Complete SOC (Start of Care) visits for ortho patients when nursing not ordered
• Educate patients and families on home exercise programs and safety
• Document patient progress accurately in HCHB (HomeCare HomeBase) EMR
• Collaborate with RNs, OTs, SLPs, and aides to ensure coordinated care
• Manage a daily caseload of approximately 5 visits within Tehama County

Qualifications

• Current California Physical Therapist license in good standing
• BLS certification required
• Minimum 1 year of experience as a Physical Therapist (home health experience required)
• Ability to travel up to 40 miles per day within service area
• Comfortable using EMR systems (HCHB experience preferred)
• Valid driver’s license and reliable transportation

Education

• Doctorate or Master's degree in Physical Therapy from an accredited program

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Knowledge of orthopedic and geriatric patient care
• Proficiency with HomeCare HomeBase (HCHB) EMR
• Excellent patient education and communication skills
• Ability to work independently and manage time efficiently
• Understanding of home health regulations and Medicare guidelines
