Roles & Responsibilities

• Provide high-quality direct patient care in the home setting, delivering individualized physical therapy interventions
• Assess patient function and safety in the home environment and develop comprehensive plans of care
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Document thoroughly and accurately using OASIS and agency documentation standards
• Evaluate, fit, and instruct patients and caregivers on rehabilitation equipment including splints, orthoses, prostheses, wheelchairs and ADL aids
• Work independently with minimal supervision while maintaining timely communication with the agency
• Participate in orientation and required non-billable training as outlined by the facility

Qualifications

• Massachusetts Physical Therapist License — must be uploaded with profile (required)
• Home Health Certification and proven home health experience — OASIS proficiency is non-negotiable
• 2+ years combined acute nursing (hospital-based) and home care experience (required)
• Current American Heart Association BLS certification (required)
• Valid driver’s license and reliable personal vehicle for home visits (required)
• Ability to work independently, manage caseloads, and communicate effectively with patients and care teams
• Supervisor-signed reference from within the past two years (required)

Education

• MA Physical Therapist License required; degree as required for licensure (e.g., DPT or equivalent)

Knowledge & Skills

• Proficiency with OASIS documentation and home health regulatory requirements
• Strong clinical assessment and care plan development skills
• Experience with rehabilitation equipment: splints, orthoses, prostheses, wheelchairs and ADL devices
• Skilled in therapeutic modalities and functional mobility training
• Excellent patient education and caregiver training abilities
• Time management and autonomous decision-making in community settings
