Location: Santa Maria, CA

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Provide occupational therapy services to patients in their homes within an assigned geographical region
- Develop and implement individualized treatment plans to improve patients' functional independence
- Collaborate with interdisciplinary team including PT, SLP, and nursing staff
- Complete OASIS assessments and document patient progress in Cerner EMR
- Educate patients and caregivers on home exercise programs and adaptive techniques
- Participate in patient care conferences and discharge planning
Qualifications
- Current California Occupational Therapist license
- BLS certification required
- Minimum one year of experience as an OT (home health experience preferred)
- Experience with OASIS documentation required
- Proficiency with Cerner EMR system
- Valid driver's license and reliable transportation
Knowledge & Skills
- Knowledge of orthopedic and neurological rehabilitation including stroke and spinal injuries
- Strong assessment and clinical reasoning skills
- Excellent communication and patient education abilities
- Ability to work independently in a home health setting
- Time management and organizational skills
