Roles & Responsibilities

• Perform comprehensive biopsychosocial assessments and initial evaluations to craft meaningful care plans
• Coordinate discharge planning and community resources to ensure safe transitions home or to post-acute care
• Document care delivery, plans and goals in Epic with clarity and timeliness
• Collaborate with internal and external teams to arrange home care, DME and transportation
• Refer patients to rehab and long-term care as appropriate and manage utilization review tasks
• Address protective services concerns including adult protective services, guardianship and power of attorney matters
• Support patients with communication barriers and culturally informed care needs
• Float between hospital units and both campuses as needed to meet patient needs

Qualifications

• Master of Social Work (MSW) with WA LISW/LCSW/LMSW/LSW approvals or equivalent RN licensure if applying as RN
• Washington clinical or associate social work license in hand, or pending WA license accepted
• BLS certification required
• Minimum 1 year of relevant experience in acute care, discharge planning or care coordination
• Experience with Epic charting strongly required
• Comfort working weekends every other weekend and floating across units and campuses

Education

• Master's in Social Work (MSW) preferred for Medical Social Worker roles
• Or current RN licensure for candidates applying as Registered Nurse

Knowledge & Skills

• Care coordination and discharge planning expertise
• Biopsychosocial assessment and care planning
• Knowledge of Medicaid, Medicare, medical transportation and community resources
• Familiarity with protective services, guardianship and advance directives
• Experience across settings: Med-Surg, ED, ICU, NICU, L&D/mother-baby
• Strong documentation and communication skills within Epic
• Ability to collaborate with interdisciplinary teams and external partners
