Roles & Responsibilities
- Provide direct patient care with compassion and skill, ensuring dignity and comfort
- Administer medications and treatments accurately and document accordingly
- Perform focused patient assessments and report changes promptly to the care team
- Collaborate with nurses, aides and interdisciplinary staff to deliver coordinated care
- Maintain clean, safe patient environments and follow infection control protocols
- Support patient and family education with clear, kind communication
Qualifications
- Valid LPN license in Pennsylvania (required)
- Current BLS/CPR certification or willingness to obtain upon hire
- Up-to-date immunizations and ability to pass background/health screenings
- Strong communication skills and a heart for patient-centered care
- Previous experience in long-term care, skilled nursing, or similar setting is a plus
Education
- Graduate of an accredited Practical/Vocational Nursing program (LPN)
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and vital signs monitoring
- Comfortable using electronic medical records and documentation systems
- Ability to prioritize tasks in a fast-paced clinical setting
- Teamwork, professionalism, and strong patient advocacy
