Roles & Responsibilities

Provide compassionate, high-quality nursing care to residents as an LPN
Administer medications and treatments accurately and document carefully
Monitor patient status and communicate changes promptly to the care team
Collaborate with RNs, physicians, and interdisciplinary staff to implement care plans
Assist with activities of daily living while promoting independence and dignity
Maintain accurate and timely clinical documentation and follow facility policies
Support quality improvement initiatives and help maintain a safe environment

Qualifications

Current LPN licensure (facility may require Pennsylvania licensure)
Completed accredited practical nursing program
Basic Life Support (BLS) certification or willingness to obtain upon hire
Recent clinical experience preferred; strong patient-care skills required
Ability to pass required background checks and meet facility onboarding requirements
Professional, patient-centered communication and teamwork skills

Education

Graduation from an accredited Practical Nursing (LPN) program
High school diploma or GED required

Knowledge & Skills

Solid knowledge of medication administration and documentation practices
Familiarity with infection control standards and patient safety protocols
Good assessment skills and ability to recognize changes in patient condition
Comfortable using electronic health records and clinical documentation systems
Strong teamwork, time management, and prioritization abilities
