Roles & Responsibilities

Provide compassionate direct patient care and perform routine assessments
Administer medications and treatments accurately and safely
Document patient care clearly in the medical record and report changes
Collaborate with nurses and interdisciplinary staff to ensure coordinated care
Support patient and family education around care plans and health maintenance
Adhere to facility policies, infection control and safety protocols

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Experience in long-term care, rehab, or a related setting is a plus
Strong communication skills and a patient-centered approach
Ability to work 8-hour shifts and adapt to the needs of the unit

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program or equivalent vocational training

Knowledge & Skills

Medication administration and accurate charting
Basic patient assessment and vital sign monitoring
Infection control practices and patient safety
Familiarity with electronic health records and documentation standards
Time management and teamwork in a fast-paced clinical setting
