Roles & Responsibilities
- Provide safe, compassionate direct patient care and support bedside needs
- Administer medications and treatments according to provider orders and facility policy
- Monitor and document patient status, vital signs, and care interventions in the medical record
- Collaborate with RNs, providers, and interdisciplinary team to coordinate patient care
- Assist with activities of daily living and support patient comfort and dignity
- Follow infection control practices and maintain a safe care environment
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania
- Current BLS/CPR certification or willingness to obtain
- Ability to pass background check and required health screenings
- Strong communication skills and a team-oriented attitude
- Comfortable with clinical documentation and basic patient assessments
Education
- Graduate of an accredited Practical Nursing (LPN) program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and medication safety
- Basic patient assessment and vital signs monitoring
- Wound care fundamentals and assistance with ADLs
- Familiarity with electronic medical records or willingness to learn
- Infection control principles and safe clinical practice
- Clear, compassionate communication with patients and families
