Roles & Responsibilities

Provide compassionate basic nursing care and daily support to residents
Administer medications and treatments accurately and safely
Monitor residents’ conditions and promptly report changes to the care team
Document resident care, observations and communications clearly and timely
Collaborate with nurses and interdisciplinary staff to promote excellent outcomes
Support resident comfort, dignity and participation in daily activities

Qualifications

Active LPN license (Pennsylvania) required
Current CPR/BLS certification preferred or willing to obtain
Clear background check and ability to pass required screenings
Strong communication and team collaboration skills
Compassionate patient-focused attitude and reliable work ethic

Knowledge & Skills

Medication administration and basic clinical assessments (vital signs, intake/output)
Basic wound care and infection control practices
Accurate clinical documentation and charting habits
Ability to work effectively in a long-term care or skilled nursing environment
Interpersonal skills for working with residents, families and the care team
