Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Assist with patient assessments, reporting changes to the care team promptly
• Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care
• Maintain accurate, timely charting and complete required documentation
• Support safe patient transfers, mobility assistance, and activities of daily living
• Contribute to a positive, professional environment that respects patients and families

Qualifications

• Active LPN license (eligible for or holding Pennsylvania LPN licensure preferred)
• Current BLS/CPR certification or willing to obtain upon hire
• Ability to pass background checks and meet facility health requirements (immunizations, TB screening, etc.)
• Strong communication skills and ability to work collaboratively with a care team
• Comfortable with medication administration, basic wound care, and clinical documentation

Education

• Licensed Practical Nurse (LPN) diploma or certificate from an accredited program
• State licensure as an LPN (PA licensure preferred)

Knowledge & Skills

• Solid foundation in clinical nursing practices and patient safety
• Medication administration and basic patient assessment skills
• Proficient charting and documentation practices
• Good communication and interpersonal skills with patients, families, and team members
• Ability to prioritize tasks and manage time during an 8.5-hour shift
