Roles & Responsibilities
- Provide compassionate, patient-centered care at the bedside
- Administer medications and treatments according to orders and policies
- Monitor and document patient vital signs and clinical changes accurately
- Collaborate closely with RNs, providers, and the care team to coordinate care
- Educate patients and families about care plans and discharge instructions
- Maintain accurate and timely documentation in the patient record
Qualifications
- Current, active LPN license in Pennsylvania (required)
- Basic Life Support (BLS/CPR) certification (preferred or obtain upon hire)
- Up-to-date immunizations and ability to pass background and drug screening
- Comfortable with electronic medical records and basic charting
- Professional communication skills and a patient-first attitude
- Previous LPN experience in a clinical or long-term care setting preferred
Education
- LPN diploma or completion of an accredited practical nursing program
- High school diploma or GED
Knowledge & Skills
- Strong knowledge of nursing fundamentals and medication administration
- Skilled in taking and interpreting vital signs and basic assessments
- Familiarity with infection control practices and safety protocols
- Good documentation practices and attention to detail
- Ability to work well in a fast-paced, team-oriented environment
- Patient education and effective communication with diverse populations
