Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in accordance with care plans
Administer medications and treatments safely and accurately
Monitor and document vital signs, behaviors, and clinical changes promptly
Collaborate with RNs, CNAs, and interdisciplinary team members to deliver coordinated care
Communicate effectively with residents and families to ensure comfort and understanding
Maintain accurate clinical documentation in the patient record
Follow infection control standards and facility policies to keep residents safe

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited LPN/licensed practical nurse program
Current CPR/BLS certification (or willingness to obtain)
Comfort with medication administration and basic nursing procedures
Strong communication skills and a caring, patient-centered approach
Previous long-term care or skilled nursing experience preferred

Education

Graduation from an accredited LPN/Vocational Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation
Basic wound care and dressing changes
Vital signs assessment and clinical observation
Familiarity with electronic health records and charting
Infection control practices and safety protocols
Teamwork, time management, and resident-centered communication
