Roles & Responsibilities

Provide compassionate, patient-centered care in collaboration with the nursing team
Administer medications and treatments according to care plans and facility policies
Monitor and document patient status, reporting changes to the RN and provider promptly
Assist with activities of daily living and promote patient comfort and dignity
Support care coordination and effective communication with patients, families, and interdisciplinary staff
Follow infection control, safety protocols, and facility procedures to maintain a safe environment

Qualifications

Current LPN license in Pennsylvania or compact/state-appropriate licensure
CPR/BLS certification (or willingness to obtain promptly)
Demonstrated ability to work well within a clinical team and communicate clearly
Prior experience in a long-term care, skilled nursing, or similar setting preferred
Up-to-date immunizations and ability to pass background and health screenings

Education

Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

Strong clinical assessment and basic nursing skill set
Medication administration and documentation accuracy
Comfortable using electronic health records and charting
Good time management and ability to prioritize patient needs
Compassionate bedside manner and strong interpersonal skills
Knowledge of infection control and patient safety practices
