Roles & Responsibilities

• Provide compassionate bedside care to residents with professionalism and heart
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments and promptly report any changes in patient condition
• Collaborate with nurses, CNAs, and the interdisciplinary team to deliver coordinated care
• Maintain clear, timely, and accurate clinical documentation in the charting system
• Support individualized care plans and engage patients and families with respect and empathy
• Adhere to infection control practices and facility safety protocols at all times

Qualifications

• Current, active LPN license in Pennsylvania or eligibility to obtain one
• BLS/CPR certification required (or ability to obtain upon hire)
• Current immunizations and TB screening per facility policy
• Strong communication skills and a patient-centered approach
• Prior experience in long-term care, rehab, or similar clinical settings preferred

Education

• Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

• Solid understanding of medication administration and basic nursing assessments
• Familiarity with electronic health records and accurate clinical documentation
• Knowledge of infection control, safety, and standard nursing procedures
• Ability to prioritize tasks, manage time effectively, and work collaboratively
• Compassionate patient care skills and the ability to educate residents and families
