Roles & Responsibilities

Provide compassionate, high-quality nursing care to assigned patients
Administer medications and treatments safely and document appropriately
Monitor and record patient vitals and clinical changes, escalating concerns
Collaborate with nurses, CNAs, and the interdisciplinary team to deliver coordinated care
Maintain accurate, timely patient records and complete required documentation
Offer patient and family education in a clear, respectful manner
Support admissions, discharges, and transitions of care smoothly

Qualifications

Active LPN license in Pennsylvania (required)
Graduate of an approved Practical Nursing program or equivalent
Current BLS/CPR certification (recommended or as required by facility)
Demonstrated clinical competence and comfort with medication administration
Strong communication and teamwork skills; professionalism and reliability
Ability to pass facility background checks and required onboarding

Education

Completion of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

Medication administration and basic clinical procedures
Patient assessment and vital sign monitoring
Wound care and basic dressing changes
Clinical documentation and charting accuracy
Time management and prioritization in a busy environment
Effective communication with patients, families, and clinical teams
