Roles & Responsibilities
- Provide compassionate direct patient care, administering medications and treatments as ordered
- Monitor and document patient vital signs and changes in condition accurately
- Collaborate with nurses and interdisciplinary team to develop and follow care plans
- Perform admissions, discharges, and transfers with professionalism and attention to detail
- Educate patients and families on care plans, medications, and safety measures
- Maintain clear, timely charting and communicate concerns to charge nurse or provider
Qualifications
- Active LPN license in Pennsylvania (or compact/state-appropriate) required
- Completion of an accredited LPN/LVN program
- Current Basic Life Support (BLS) certification or willingness to obtain
- Strong clinical judgment, reliability, and a caring bedside manner
- Ability to pass background check and meet facility onboarding requirements
Education
- Graduate of an accredited LPN/LVN program
Knowledge & Skills
- Comfortable with medication administration and wound care
- Skilled in patient assessment and documentation
- Good communication and teamwork with nursing staff and providers
- Familiarity with electronic charting systems or willingness to learn
- Time management and prioritization in a busy clinical environment
- Infection control and safety best practices
