Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments safely and accurately
Monitor patient status, document changes, and escalate concerns promptly
Collaborate with nurses, therapists, and the care team to deliver coordinated care
Support resident comfort and dignity through daily personal care assistance
Maintain accurate records in the facility charting system and follow facility policies

Qualifications

Current Licensed Practical Nurse (LPN) license; ability to meet state licensure requirements
Basic Life Support (BLS) certification or willingness to obtain
Relevant experience in long-term care, skilled nursing, or similar preferred
Current immunizations and ability to meet facility onboarding requirements
Strong communication skills and a patient-centered approach

Education

Completion of an accredited LPN program or equivalent vocational training
High school diploma or GED

Knowledge & Skills

Medication administration and basic nursing procedures
Vital signs monitoring and clinical assessment
Resident-focused care and activities of daily living support
Clear documentation and electronic charting familiarity
Team collaboration, time management, and reliable follow-through
