Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments according to care plans and policies
- Perform thorough documentation of patient status, interventions, and outcomes
- Monitor patients for changes and promptly communicate concerns to the care team
- Support patient admissions, discharges, and transitions with attention to dignity and safety
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Participate in quality improvement and infection control practices to keep everyone safe
Qualifications
- Current LPN license (PA licensure preferred or eligibility to obtain)
- Completion of an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
- Comfortable with medication administration, basic assessments, and charting
- Clear background check and up-to-date immunizations as required by facility
- Positive attitude, strong communication skills, and a team-first mindset
Education
- High school diploma or GED
- Completion of an accredited LPN / Practical Nursing program
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Solid documentation practices and familiarity with electronic or paper charting
- Strong patient assessment and clinical observation abilities
- Knowledge of infection control and standard precautions
- Excellent interpersonal skills for working with patients, families, and colleagues
- Ability to prioritize tasks in a fast-paced care setting
