Contract Type: Per-Diem

Shift Type: 4-hour shift (start/end times to be determined), days to be determined — LPN shift

Candidate Pay Rate: $51 per hour

Roles & Responsibilities

Provide compassionate, direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and safely
Monitor patients’ conditions and report changes promptly to nursing leadership
Document care clearly and thoroughly in the patient record
Collaborate with RNs, providers, and interdisciplinary team members to support patient plans
Support residents with activities of daily living and promote dignity and comfort

Qualifications

Active LPN license (practical nurse) — ability to practice in Pennsylvania preferred
Current Basic Life Support (BLS) certification or willingness to obtain
Comfortable with medication administration, basic wound care, and patient assessment
Strong communication skills and a team-oriented mindset
Previous experience in long-term care or skilled nursing preferred but new grads welcome

Education

LPN diploma, certificate, or equivalent practical nursing education

Knowledge & Skills

Patient-focused clinical skills for assessment and basic interventions
Safe medication administration and documentation practices
Familiarity with electronic charting or willingness to learn
Good infection control practices and attention to detail
Empathy, patience, and the ability to communicate with residents and families
