Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, direct patient care including medication administration and assistance with activities of daily living
Perform regular assessments and accurately document resident condition and care in the chart or EMR
Implement care plans and communicate changes to the interdisciplinary team to ensure continuity of care
Support wound care, treatments, and follow established protocols to promote resident comfort and healing
Participate in safe and thorough shift handoffs and contribute to a positive workplace culture
Educate residents and families about care plans and daily routines in a clear, respectful way

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Current CPR/BLS certification (required)
Graduate of an accredited practical nursing program
Previous experience in long-term care or a related setting preferred
Ability to pass facility-required background and drug screening

Education

Practical Nursing diploma or LPN certificate
Completion of an accredited LPN program

Knowledge & Skills

Medication administration and accurate charting
Vital signs, basic assessments, and monitoring changes in condition
Basic wound care and dressing changes
Familiarity with electronic medical records and documentation best practices
Strong communication, teamwork, and resident-centered care approach
