Roles & Responsibilities

• Provide compassionate direct patient care in accordance with facility policies
• Administer medications and treatments safely and accurately
• Monitor patient status, report changes to the care team, and document findings thoroughly
• Assist with ADLs (activities of daily living) while promoting patient dignity
• Collaborate with nurses and interdisciplinary team members to support care plans
• Maintain accurate and timely charting to ensure continuity of care
• Support infection control and safety practices to protect patients and staff

Qualifications

• Current LPN licensure (Pennsylvania licensure strongly preferred)
• Completed accredited LPN program or equivalent practical nursing education
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Strong communication skills and ability to work as part of a clinical team
• Prior experience in a long-term care or similar setting preferred but new grads considered

Education

• Graduate of an accredited LPN / practical nursing program
• High school diploma or GED required

Knowledge & Skills

• Solid fundamentals in medication administration and basic nursing procedures
• Comfortable with patient assessment and monitoring
• Good documentation practices and familiarity with electronic charting or willingness to learn
• Ability to follow care plans and escalate clinical concerns appropriately
• Compassionate bedside manner and strong interpersonal skills
• Time management and the ability to prioritize in a busy care environment
