Contract Type: Per-Diem

Shift Type: AP, 6:30 PM - 6:30 PM

Candidate Pay Rate: $17 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to patients — assist with activities of daily living like bathing, dressing, and mobility
Monitor and report changes in patient condition to the nursing team promptly
Take and record basic vital signs accurately and consistently
Support rehabilitative activities and follow care plans developed by nurses and therapists
Maintain clear, timely documentation in patient records
Follow infection control and safety protocols to protect patients and staff
Collaborate with interdisciplinary team members to ensure patient-centered care

Qualifications

Current CNA certification (required)
Completion of an accredited CNA program or equivalent training
Comfortable providing direct patient care in a rehab/long-term care setting
Strong communication skills and a team-first attitude
Ability to pass background check and any required health screenings

Education

High school diploma or GED preferred
Completion of an accredited CNA training program

Knowledge & Skills

Fundamentals of patient care and assistance with ADLs
Basic vital signs monitoring and accurate documentation
Infection control practices and facility safety procedures
Clear, compassionate communication with patients, families, and team members
Ability to follow care plans and take direction from nursing staff
