Roles & Responsibilities

• Process insurance authorizations accurately and efficiently to keep care moving
• Coordinate post-acute referrals and arrange safe transitions home or to facilities
• Deliver Medicaid notices to patients and document outreach in Epic
• Collaborate with internal and external care teams to ensure continuity of care
• Document pertinent information in Epic to facilitate care coordination and follow-up
• Educate patients and caregivers to support safe discharges and next steps
• Refer patients to rehab, long-term care, community resources, and other disciplines as needed
• Support benefits eligibility screening, prior authorizations, and pre-cert reviews

Qualifications

• Minimum 1 year of relevant experience (1 year required as listed)
• Epic experience required — you will be charting in Epic daily
• MSW preferred but not required; candidates with relevant experience are welcome
• State license is not required for this role
• First-timers accepted — we welcome motivated early-career applicants
• Comfortable with clerical/coordination tasks in a fast-paced acute care setting
• Able to complete a 40-hour orientation and collaborate with interdisciplinary teams

Education

• MSW preferred
• Relevant degree, diploma, or equivalent experience in social work or related field preferred

Knowledge & Skills

• Strong Epic/charting skills and attention to documentation detail
• Knowledge of discharge planning and care coordination processes
• Familiarity with Medicaid and Medicare processes and community-based programs
• Experience with prior authorizations, benefits eligibility, and referral coordination
• Good communication and interpersonal skills for patient/caregiver education
• Ability to coordinate services to home care and equipment following discharge
• Comfortable working in short-term acute care and case management settings
