Roles & Responsibilities

• Evaluate patients’ functional abilities and occupational needs to create person-centered care plans
• Develop and implement therapeutic interventions that maximize independence and safety
• Document treatment progress clearly and timely in accordance with facility and CRG policies
• Collaborate with nurses, PT/SLP, social work and the interdisciplinary team to coordinate care
• Meet productivity expectations and contribute to a high-performing therapy department
• Follow CRG policies & procedures and maintain professional standards, including dress code

Qualifications

• 1-3 years of Occupational Therapy experience (as listed in the job)
• Active Occupational Therapist licensure in New Mexico or eligibility to obtain licensure
• Experience in a Skilled Nursing Facility (SNF) or similar post-acute setting preferred
• Strong clinical assessment, treatment planning and documentation skills
• Excellent communication and teamwork abilities to thrive in a collaborative therapy team
• Willingness to follow CRG policies & procedures and meet productivity requirements

Knowledge & Skills

• Skilled in occupational therapy evaluation and interventions for adult/geriatric populations
• Familiarity with SNF practice setting, regulatory documentation and productivity expectations
• Good time management and organizational skills to balance caseload and documentation
• Comfortable working as part of an interdisciplinary team and adapting to facility protocols
• Basic computer skills for electronic documentation and scheduling
• Professional presence (black scrubs dress code for this facility)
