Roles & Responsibilities

• Process insurance authorizations and prior authorizations accurately using Epic
• Coordinate post-acute care referrals and arrange transitions to home health, rehab, and long-term care
• Deliver Medicaid notices to patients and document communication clearly
• Collaborate with interdisciplinary partners (PT, pharmacy, interpretation services, social services) to support discharge plans
• Document case management activities in Epic to ensure continuity of care
• Educate patients and caregivers to facilitate safe, informed discharges
• Support utilization review tasks like pre-cert reviews and benefits eligibility checks
• Maintain organized records and follow facility guidelines around observed holidays and orientation

Qualifications

• Minimum 1 year of relevant experience (job accepts first-timers as noted)
• Epic experience is required — you’ll be charting here a lot
• Clinical acute care background helpful but not required
• MSW preferred (but not mandatory) for clinical/social work-focused roles
• Comfort working Monday-Friday daytime schedule and handling a caseload (average 18–24 patients)
• Strong documentation, communication, and care coordination skills
• Ability to follow RTO submission requirements at time of submittal

Education

• MSW preferred
• Clinical background helpful but not required

Knowledge & Skills

• Knowledge of case management and care coordination practices
• Experience or familiarity with prior authorization and pre-cert processes
• Understanding of discharge planning and arranging home care/DME
• Familiarity with Medicaid and Medicare basics and community-based resources
• Proficiency with Epic charting and clinical documentation
• Ability to collaborate with internal and external team members and community partners
• Organizational skills to manage post-acute referrals and patient notices
