Roles & Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans that promote independence
• Deliver skilled therapeutic interventions focused on mobility, strength, pain management, and functional transfer training
• Document thoroughly and timely in EPIC/OASIS to ensure continuity of care and compliance
• Teach patients and caregivers home exercise programs and safety strategies to support long-term outcomes
• Coordinate with interdisciplinary team members and referring clinicians to optimize patient progress
• Manage a typical daily schedule of up to 6 patient visits while meeting productivity expectations
• Maintain safe driving and time management while traveling between patient homes

Qualifications

• Minimum 1 year of skilled Home Health physical therapy experience required
• Current State Physical Therapy license (pending license accepted)
• BCLS (required)
• Experience with OASIS charting and EPIC documentation (OASIS experience required; please call out if missing)
• Must have reliable personal transportation and willingness to travel within assigned patient radius (occasional 50+ mile visits)
• Primary COVID vaccination series plus one booster required by facility
• First-time travelers are not accepted for this role

Knowledge & Skills

• Home Health clinical expertise and home exercise program design
• Proficiency with OASIS and EPIC documentation systems
• Orthopedics: total hip and knee replacement post-op care and mobility training
• Neurologic rehab: stroke, spinal cord injury, traumatic brain injury, cognitive disorders
• Assessment skills, body mechanics, and therapeutic exercise prescription
• Patient and family education with strong communication skills
• Ability to work independently, prioritize visits, and meet productivity expectations
