Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility setting
• Assess patient function and create individualized treatment plans that promote recovery and independence
• Document patient progress clearly and timely in accordance with facility and CRG policies
• Collaborate closely with the therapy team and interdisciplinary staff to deliver cohesive care
• Meet productivity expectations and contribute to team goals (therapist productivity requirement 90%)
• Follow CRG policies and procedures and maintain professional presence on-site to secure guaranteed hours

Qualifications

• PT or DPT (listed as required job skills)
• 1-3 years of clinical experience preferred (per job posting)
• Experience or comfort working in a Skilled Nursing Facility (SNF) practice setting
• Comfort adhering to facility policies and meeting productivity standards (90% productivity requirement)
• Willingness to be present on-site during scheduled hours and follow dress code of black scrubs
• Able to participate in an interview with the Regional Director of Therapy as requested

• Strong documentation and clinical charting practices
• Ability to collaborate with rehab teams and interdisciplinary staff
